# `review_already_decided` — Review already decided

**HTTP 409 Conflict**

Message: "Review was already <approved|denied>."

## What causes it
- A decision was already recorded for this execution, by the API, the operator UI, or a bulk action. Decisions are final.

## How to fix it
1. Read the recorded outcome from `review_status` and `review_events`; do not resend.
2. If the outcome was wrong, re-evaluate the applicant with `POST /executions/{id}/re_evaluations`, which produces a fresh execution and a fresh review.
